We participate in many major health plans and our office will file your insurance claim as a courtesy. Please keep in mind that your insurance company and you are responsible for any remaining balance. We cannot guarantee that your particular policy will cover the service you require.
Most insurance plans include expenses which must be paid by the patient. If your plan requires a co-payment, we require that this be paid at the time of your appointment. We require that a credit, debit or ACH authorization be on file prior to treatment. This allows us to collect the patient due balance in a timely and efficient manner.
